Total Laparoscopic Hysterectomy (TLH)

What Is It?

A Total Laparoscopic Hysterectomy (TLH) is a minimally invasive surgical procedure to remove the entire uterus, including the cervix, using small keyhole incisions in the abdomen.

This advanced technique uses a laparoscope—a thin tube with a camera—to guide the surgery, allowing for precise removal with less pain and faster recovery compared to traditional open surgery.

Is TLH Recommended?

TLH may be advised for conditions such as:

  • Uterine fibroids causing pain or heavy bleeding
  • Adenomyosis (thickening of the uterine wall)
  • Endometrial hyperplasia or early-stage cancer
  • Abnormal uterine bleeding not responding to other treatments
  • Pelvic pain linked to uterine causes
  • Uterine prolapse in select cases

 How Is It Performed?

  • Under general anaesthesia, 3-4 small incisions (usually 0.5–1 cm) are made in the lower abdomen.
  • A laparoscope is inserted to provide a magnified view of the pelvic organs.
  • Specialized instruments are used to carefully detach the uterus from surrounding tissues.
  • The uterus is removed either through the vagina or in small pieces through the keyhole incisions.
  • The incisions are closed with dissolvable stitches.

 Risks and Considerations

While TLH is generally safe, potential risks include:

  • Bleeding or infection( rare)
  • Injury to bladder, bowel, or blood vessels (Very rare)
  • Anaesthesia complications
  • Formation of adhesions
  • Rarely, need for open surgery if complications arise
